The cost estimates on this site are intended for individuals who are uninsured or covered by private insurance. They are generally not relevant to those covered by Medicare and Medicaid. Cost estimates reflect healthcare professional fees and are not the negotiated rates which may be available through insurance plan networks.

Procedural CPT Code

As is the case with many healthcare costs, charges for medical services tend to vary from region to region. In this field, enter the location where you will be receiving or have received medical care. You can indicate the location by zip code or by entering a city and state. All of the following are examples of valid entries in this field:

We will use this information to give you results that are specific to your provider’s geozip. A geozip is a geographic area that is usually defined by the first three digits of a zip code. For example, the geozip for zip code 13202 is 132. Because our cost estimates are based on geozips, you will generally get the same results for zip codes that begin with the same first three digits.

In this field, enter a word or phrase related to the medical procedure for which you would like to estimate your costs. Every procedure has a Current Procedural Terminology (CPT) Code associated with it. If you know the code for your procedure, you may enter that instead of a word or phrase. You may also enter a condition such as “diabetes.” When you enter a condition, the system will show you a list of conditions that match your search.

Using the FH Medical Cost Lookup

The FH Medical Cost Lookup offers information on estimated costs for healthcare professionals’ services for medical procedures, including a limited number of anesthesia services available on FH Estimate Assist®. At this time the website does not include charges for durable medical equipment or separate facility fees, which may be billed in addition to healthcare professional fees, when services are performed in a hospital, freestanding clinic, or other treatment center. Over time, the FH Medical Cost Lookup will be expanded to include this information.

Estimating Your Costs

The medical cost lookup uses data from the FAIR Health Database to estimate costs for specific medical procedures. You can use the site to estimate your out-of-pocket costs with and without insurance coverage.

If you are insured, please note that some plans set out-of-network reimbursement rates based on a percentage of “usual, customary, and reasonable” (UCR) charges, while others use a formula based on a fee schedule established for Medicare.

Insured out-of-pocket costs will be estimated using the UCR-based reimbursement method. If your plan bases reimbursement on a percentage of Medicare, you can easily recalculate your results by selecting the “Medicare-Based” button on the results page.
